ACR Image Competition 2023 Results, Part 3—People’s Choice Winner

Rajat Ranka, MD  |  Issue: February 2024  |  February 9, 2024

For the 2023 Image Competition, the ACR sought images representing a diverse range of patients with idiopathic inflammatory myopathies (IIMs) or IIM mimics.

Chronic Facial Ulcers in Anti-Melanoma-Differentiation-Associated Gene 5 (MDA-5) Antibody Amyopathic Dermatomyositis

These images depict a 27-year-old patient who developed erythematous violaceous lesions over his upper chest, face and scalp over six months. Such lesions are associated with photosensitivity, non-scarring alopecia, oral ulcers, heliotrope rash and Gottron papules. Progressive ulcers over the bilateral malar areas and the left angle of the mandible followed. Ulcer edge biopsy showed perivascular and peri-adnexal lymphoid infiltrates, vacuolar interface dermatitis and basement membrane zone thickening, with no clinical, radiological or laboratory evidence of myositis, interstitial lung disease or arthritis.

The patient was positive for the anti-melanoma-differentiation-associated gene 5 (MDA-5) antibody and was diagnosed with amyopathic dermatomyositis. Anti-MDA-5 antibody is usually associated with digital and extensor ulcers; however, facial ulceration, as in this case, is rare.

Anti-MDA-5 antibody positivity is associated with often severe and rapidly progressive interstitial lung disease (ILD). Thus, all patients with anti-MDA-5 antibody should be evaluated with pulmonary function testing and high-resolution computed tomography imaging of the chest. It’s important to carefully evaluate these patients for muscle involvement, which can be subtle.

About the Contest

The Rheumatology Image Library is a highly accessed teaching resource and images from the ACR’s annual contest are added to it, keeping the library dynamic and current. In addition to the Best Overall Image in 2023, winning images were selected from four regions around the world. Look for the People’s Choice and regional winners in future issues and on our website.

Since it was first reported in 1916, a correlation between inflammatory myopathies and cancer has been noted in several studies. Population studies have confirmed this relationship, and the phrase cancer-associated myopathy has entered the vernacular. Over the past decade, research efforts have shifted toward revealing associations between autoantibodies and clinical phenotypes. One subset of auto-antigens…

The idiopathic inflammatory myopathies (IIM) encompass eight categories: 1) dermatomyositis (DM) in adults, 2) juvenile dermatomyositis, 3) amyopathic DM, 4) cancer-associated DM, 5) polymyositis, 6) immune-mediated necrotizing myopathy, 7) inclusion body myositis, and 8) overlap myositis.1 These categories help classify the myopathies based on clinical and histologic features. The incidence of IIM is estimated at…

Clinically amyopathic dermatomyositis (CADM), a rare subset of dermatomyositis (DM), is an autoimmune disease characterized by cutaneous findings of typical DM without evidence of myositis. Childhood presentation of CADM is rare, and not many studies describe the epidemiology of juvenile CADM.1,2 Although lung disease is rare among patients with juvenile DM, a few reports have…

    Autoantibodies in Autoimmune Myopathy

    September 18, 2017

In recent years, scientists and clinicians have learned a great deal about autoantibodies occurring in idiopathic inflammatory myopathies (IIMs). These new discoveries have reshaped our understanding of distinct clinical phenotypes in IIMs. Scientists continue to learn more about how these autoantibodies shape pathophysiology, diagnosis, disease monitoring, prognosis and optimum treatment. Moving forward, these autoantibodies will…
