Rheumatology Coding Corner Question: Billing for an Inflectra Infusion

A 68-year-old female patient returns to the office for a second round of Inflectra (infliximab-dyyb) infusion for her rheumatoid arthritis with rheumatoid factor of multiple joints without organ or systems involvement. She reports pain, stiffness and swelling in her left and right knees and right elbow and rates the pain at a 9 on a 10-point scale. She currently takes an NSAID and methotrexate 15 mg once weekly. She has no known allergies and denies any other complaints.

Upon examination, her temperature is 98°F, weight is 135 lbs., and blood pressure is 110/75.

The patient is given a premed push of 50 mg of diphenhydramine and then infused with 200 mg of infliximab-dyyb for a total of 1 hour 43 minutes. The patient’s medication is left unchanged.

Note: The infusion began at 11:05 a.m. and ended at 12:48 p.m.
