Recognizing Physician Burnout, & Tips to Fight It

Philip Seo, MD, MHS  |  February 17, 2018

4 Patients in 4 Weeks Baltimore is a little over two hours away from Richmond, Va., by car. I know this now because I recently drove to Richmond to attend a memorial service. I drove in silence. Music made me sleepy, and I could not bear to listen to another iteration of how we are…

One Member’s Personal Story Illustrates How RheumPAC Can Help Underserved Children

Brett Smith, DO  |  November 1, 2017

Almost one year into my term on the ACR’s Political Action Committee, RheumPAC, I’m looking back and reflecting on what led me here. My early career was focused on learning the basics of clinical practice and building relationships with healthcare providers. Shortly after I completed my fellowship, I established goals to improve access to care…

Hemoptysis in a Young Indian Male

Payam Pourhassani, DO, MSc, Sneha Patel, MD, & Arundathi Jayatilleke, MD  |  September 19, 2017

A 22-year-old Indian male presented to the emergency department with hemoptysis. A month prior, he had presented to an urgent care center complaining of cough with occasional episodes of blood-tinged sputum in the morning. He was diagnosed with community-acquired pneumonia based on a chest X-ray without laboratory testing and was prescribed levofloxacin. A few days…

Changes in BMI Associated with Improvements in Disease Activity & Glucocorticoid Treatment

Arthritis Care & Research  |  July 4, 2017

A recent study examined the relationship between increased BMI and antineutrophil cytoplasmic antibody–associated vasculitis treatment with glucocorticoids. The results: Weight gain was independently associated with reductions in disease activity, increased glucocorticoid exposure and randomization to rituximab. The most significant increases in BMI occurred during the first six months of treatment, and newly diagnosed patients were more likely to experience an increase in BMI…

Draft Criteria for ANCA-Associated Vasculitis Released

Susan Bernstein  |  March 20, 2017

WASHINGTON, D.C.—Rheumatologists do not have up-to-date diagnostic and classification criteria for vasculitis, a group of diseases that involves inflammation of the blood vessels, said experts speaking at the 2016 ACR/ARHP Annual Meeting session, Classification Criteria for ANCA-Associated Vasculitis. The most recent classification criteria were published by the ACR in 1990, and since then, new disease…

Research Offers Insight into Diagnosis, Treatment of Small-Vessel Vasculitis

Susan Bernstein  |  March 20, 2017

WASHINGTON, D.C.—Experts speaking at the 2016 ACR/ARHP Annual Meeting session, Update on Small-Vessel Vasculitis, offered insight into the latest approaches to the diagnosis and treatment of diseases involving the inflammation of blood vessels. “Vasculitis is an immune-mediated process. White blood cells invade the vessel wall, causing inflammation throughout the vessel wall,” said Jason M. Springer,…

Diffuse Alveolar Hemorrhage Secondary to Antineutrophil Cytoplasmic Antibody–Associated Vasculitis: Predictors of Respiratory Failure and Clinical Outcomes

Arthritis & Rheumatology  |  June 1, 2016

A variety of immune and nonimmune pathophysiologic mechanisms causing the disruption of alveolar capillaries with bleeding into the alveolar spaces can lead to diffuse alveolar hemorrhage (DAH). Immune-mediated mechanisms can cause DAH with or without the presence of capillaritis. The antineutrophil cytoplasmic antibody (ANCA)–associated vasculitis (AAV) syndromes—granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA)—are the…

Case Report: Interstitial Lung Disease with Positive ANCA Test

Sara Tedeschi, MD  |  October 13, 2015

“Worst of all is the pain in my calves,” she said. “It feels like burning deep inside.” So began my first encounter with a 69-year-old woman who was referred to rheumatology clinic for evaluation of two months of constitutional symptoms and a positive ANCA test, which had been ordered by her primary care doctor. Her…
